GAFC, or Group Adult Foster Care, is a MassHealth sponsored Personal Care Attendant (PCA) Program. GAFC provides assistance with personal care services to eligible individuals, aged twenty-two years and older, with disabilities.

Personal Care Attendant (PCA) Services

  • Feeding
  • Bathing
  • Toileting
  • Personal Hygiene
  • Dressing and Grooming
  • Ambulation and Transfers
  • Meal Preparation
  • Shopping and Errands
  • Laundry and Housekeeping
  • Assisting with cognitive or behavioral needs
  • Maintaining participant safety 24/7
  • Cueing or reminders to take medications
  • Supporting services e.g., doctor’s appointments, transportation

Eligibility

  • MassHealth standard or affiliated Managed Care Entity e.g CCA, Fallon etc.
  • Must be 22 years of age or older
  • Need daily assistance with one or more personal care tasks such as bathing, dressing, or toileting
